McClarence quest list

Can someone please post a list of all the McClarence quests along with the number of extractions needed to activate each quest? Also, is the requirement for unique mods or just quantity? In other words, can I use Astras and keep extracting from the same equipment?

Here is a compiled list. Extractions do not need to be unique - you could technically give him 100s of the exact same type and it would count towards progression.
